Need health insurance? Come to the Cover Oregon Application Fair to get help applying for health insurance. By using the Cover Oregon application, you can find out if you qualify for no-­‐cost health insurance or help paying your health insurance premium. DATE: Thursday, January 30, 2014 TIME: 2:00 p.m. to 7:00 p.m. Location: SMYRC - A Program of Q Center (2450 NE Sandy Blvd, Suite 100 - Portland, OR 97232) All are welcome. If you have the following documents please bring them: • Social Security number, residency card or immigration documentation for everyone who is applying for health insurance. • Income documents or self-­‐employment records such as your last tax return, most recent pay stub, financial statements or documentation of other sources of income. • Insurance plan information (for each person on the application) about any current health coverage or health insurance available through an employer.
